Show username field

I don't think it adds a lot of value when the computer username is the same as VPN username, but might confuse or make entering a long name more difficult for some users.

Ok cool! I've never used username and password before, only certificates, so didn't know how it was. Just started testing TLS + LDAP user auth and saw it.
